One of the four forms of review within the United Church of Christ is the Annual Information Review. This helps the Commission on Ministry and the Southeast Conference know where the Authorized Ministers are within this conference, what ministry they are engaged in, and how the Commission on Ministry might assist them. The deadline is February 28, 2023.
There are two ways you can return your Annual Review:

- Alternatively, you can download a PDF of the form, print and mail it to the Conference Office: PO Box 1950 Decatur, Georgia 30031.
Your information will be kept confidential and only used to update Conference and National records and evaluate how best to assist you in your ministry. You have the option to have your contact information included in the Yearbook and Access UCC as well. If you do not wish to be included, please be sure to check the appropriate boxes on the form.
